Pariva Health

Project Description
A support platform for families with special-needs children — clinicians, families, and community in one ecosystem, replacing a manual program lifecycle.

Problem
Carving Futures ran the Pariva program lifecycle manually, which made onboarding, assessments, appointments, and reporting tedious and complex for both the Pariva team and the families they support.
- Problem 1 The entire program lifecycle — onboarding, assessments, appointments, and reporting — was run manually.
- Problem 2 Families had no reliable way to track progress, access reports, or stay updated between sessions.
- Problem 3 Clinicians, managing staff, and families worked across disconnected processes with no shared system.

Solution
We engineered a comprehensive ecosystem of three connected portals to streamline operations and deepen family engagement across the program.
- Solution 1 Horizon Portal: patient onboarding, assessments, appointment bookings, and report sharing for clinicians and managing staff.
- Solution 2 Family Connect Portal: families track progress, access reports, share feedback, and receive appointment updates.
- Solution 3 Pariva Community: group creation, forum discussions, chats, and blog contributions that foster community support.

Design Process
From discovery through delivery: research interviews, information architecture, wireframes, hi-fidelity UI, and developer handoff. Each gate was validated before the next began.

Style Guide & Components
A tuned design system (type scale, colour, and component library) built to keep the experience consistent across every surface.

The Result
The platform transformed the program lifecycle — simplifying complex processes for the team and giving families regular updates and a direct channel to share feedback.
